Medicare and major HMO/PPO Plan Participants

We have a well-trained staff who understands your benefits and will work with you to ensure that your plan benefits are maximized. We do require that co-pays, co-insurance and unmet deductible amounts be paid at the time of service. If your plan requires a referral from your primary care physician, you will need to have your referral prior to arriving at our office. If you do not have insurance, or have a plan that we do not participate with, we require payment on the day of services.

Under special circumstances, we will make payment arrangements, but it is requested that this be done at the time the appointment is scheduled or during check-in rather than check-out.

Insurance participation falls into either participation with full benefits, partcicpation with reduced benefits or no participation. If you have questions about our participation, call your insurance provider with our tax ID # 56-1164316.
